Prison and High-Risk POI Guide

Not every building in Knox County is worth visiting. The prison, orphanage, sanatorium, and similar high-density locations promise high-value loot but deliver lethal zombie concentrations that can end a run in seconds. Build 42.20 added or reworked several of these POIs as part of the map glow-up, and basement systems add vertical risk layers that previous guides did not cover. This page is the tactical field reference: entry strategy, loot targets, zombie behavior, and evacuation routes for the locations that kill careless survivors.

The Golden Rule of High-Risk POIs

These buildings are not loot runs. They are controlled tactical operations. You enter with a plan, exit with what you can carry, and you never improvise inside. If you enter the prison without knowing your evacuation route, you have already made a mistake.

Prison: Maximum Security, Maximum Danger

The prison holds the highest zombie concentration in Knox County. Cell blocks, corridors, and common areas contain concentrations that no early-game character can fight with standard combat. The prison is an endgame loot objective only—which means you have a reliable vehicle, appropriate weapons, and at least two planned evacuation routes.

Entry Approach

Do not enter through the main gate. Approach from the rear perimeter where wall damage or drainage access creates a lower-traffic entry point. Scout the exterior first with binoculars or a long-range vantage point. Count zombie silhouettes in windows. If you see more than fifteen zombies visible from outside, wait or abort.

The main gate approach road is exposed. Use vehicle cover to advance, then abandon the vehicle at the perimeter—never leave a running engine near the entrance because sound draws zombies from the entire complex.

Interior Layout Knowledge

Prison interiors in Build 42.20 feature:

  • Cell blocks: Narrow corridors with cell doors. Extreme zombie concentration here because walls prevent normal pathing. Fighting in cell block corridors is a last resort.
  • Administrative building: Often fewer zombies but better loot—office supplies, computers, and rare items in filing cabinets.
  • Guard towers: Elevated sniper positions useful for drawing zombies toward a choke point before retreating. Clear tower access only if the tower itself is the objective.
  • Yard and perimeter: Open ground between buildings. The yard connects structures but offers minimal protection. Cross during daylight when visibility advantage outweighs exposure.

Loot Priorities

The prison administrative building is the primary objective:

  • Tactical equipment: Riot gear, handcuffs, and equipment spawn in armory-adjacent sections.
  • Weapons: Firearms and ammunition in armory sections if accessible.
  • Medical supplies: Infirmary within the penitentiary complex may contain high-level medical items.
  • Documents and keys: Filing cabinets contain keys that unlock other areas; carry what you can.

Do not try to clear the entire cell block for loot. The time and risk do not justify container density. Clear what is accessible from perimeter rooms and administration, then leave.

Prison Escape Protocol

Prison escape is typically exiting the way you entered. If that route is compromised, pre-stage a secondary vehicle outside the perimeter. Never enter the prison without a vehicle staged outside—running on foot through the surrounding area after a bad exit is a death sentence.

Orphanage: Institutional Horror

The orphanage is not mechanically dangerous like the prison. Its threat comes from emotional disorientation and layout confusion. Institutional multi-room buildings with identical corridors cause players to lose their exit route. Zombie groups form in rooms where echoing attracts additional undead.

Entry Approach

The exterior of the orphanage typically has lower zombie concentration than the prison, but interior rooms are connected in ways that cause surprise encounters. Enter during daylight. Mark your entry point on the map mentally or physically.

Approach from the least-visible angle. The main entrance on a street exposes you to road traffic. Use side or rear entrances when available.

Interior Strategy

Orphanages typically contain:

  • Dormitory wings: Large rooms with multiple beds and containers. Moderate zombie concentration.
  • Kitchen and cafeteria: High container density, potential fire risk if cooking equipment still has gas.
  • Administrative offices: Smaller rooms, lower zombie density, higher-value loot.
  • Basements: If the orphanage has a basement per Build 42.20 basement rules, treat it as a separate floor—verify before marking the building as cleared.

Clear methodically. Close doors behind you as you advance so you can track which rooms are cleared. Never leave a door open that you passed through without checking if you are prepared to proceed.

Loot Profile

Orphanages do not contain exceptional loot volume but often contain:

  • Clothing: Children’s clothing has no gameplay value, but adult-sized clothing from staff rooms may be present.
  • Tools: Maintenance rooms contain generic tools useful for base construction.
  • Medical supplies: Infirmaries sometimes contain bandages and basic medication.
  • Furniture: Some containers contain furniture components useful for carpentry.

The primary reason to enter an orphanage is map knowledge and exploration rather than loot density.

Sanatorium: Medical Treasure and Corridor Horror

The sanatorium offers some of the best medical loot in the game but features long corridor-heavy layouts that favor zombie pathing over player advantage. Fighting in a sanatorium is sustained corridor combat that consumes resources and attracts additional undead.

Entry Approach

Approach the sanatorium from the side opposite the main entrance. Most players enter through the front, creating noise that propagates through the entire connected structure. A side entrance limits initial contact and lets you clear an adjacent wing before the main structure notices your presence.

Do not try to clear the entire sanatorium in one run. Select one wing, clear it completely, and leave. Return on a later run for additional wings only after the first wing is fully secured.

Interior Strategy

Sanatorium layouts in Build 42.20 feature:

  • Long corridors: Zombie pathing is linear in corridors. Use this by fighting in door frames where you control engagement angles.
  • Patient rooms: Individual rooms off main corridors typically contain 1-3 zombies. Clear with shove-stomp discipline.
  • Treatment rooms: Larger spaces with multiple entry points. Approach from the single-entry side when possible.
  • Basements: If present, basements follow standard Build 42.20 basement rules with potentially higher-level medical containers.

Loot Priorities

Medical supplies are the objective:

  • Surgery rooms: Highest-level medical items including surgical equipment and rare medication.
  • Pharmacy: If accessible, contains compounding materials and specialized medication.
  • Patient rooms: Standard medical supplies—bandages, antiseptic, suturing material.
  • Storage rooms: Bulk medical storage with containers and pre-assembled kits.

Evacuation Protocol

If the main entrance becomes compromised, the sanatorium side exit through which you entered remains the primary evacuation route. Clear this route before advancing deeper into the structure. Mark cleared rooms with an object at the door threshold so you can identify your path when visibility is reduced.

General High-Risk POI Rules

These rules apply to all high-density locations:

Before Entry

  1. Stage a vehicle with the engine off within visual range of the entrance.
  2. Identify two evacuation routes and mark them mentally.
  3. Evaluate zombie density visible from outside. Abort if density exceeds manageable levels.
  4. Ensure you have at least one backup weapon and adequate medical supplies.
  5. Never enter alone on a multiplayer server unless teammates are staged outside as backup.

During Operation

  1. Move deliberately, not quickly. Speed creates noise. Deliberate movement with door checking prevents surprise encounters.
  2. Close doors behind you. Every closed door is a barrier between you and zombies you have not yet met.
  3. Mark cleared rooms. Place a visible object at the entrance of cleared rooms.
  4. Maintain situational awareness. Track which direction you entered from and which rooms remain uncleared.
  5. Exit immediately if panic rises to critical levels. No loot is worth dying for.

After Operation

  1. Return to your staged vehicle via the predetermined route.
  2. Drive at least three city blocks before stopping to evaluate acquired loot.
  3. Treat wounds immediately. Infection from scratches sustained inside high-risk POIs is common.
  4. Do not return to the same location in the same in-game week unless you left a cleared path that zombies have not reseeded.

FAQ

Frequently Asked Questions

Quick answers to the most common questions.

Is the prison worth visiting in Build 42.20?

The prison offers high-value loot including tactical gear and weapons, but zombie density is extreme. Enter only with endgame combat ability, appropriate weapons, and at least two planned evacuation routes.

What is the safest entry point for high-risk POIs?

Avoid main entrances that concentrate zombie attention. Use side or rear entry points when available. For the prison, approach from the rear perimeter. For the sanatorium, enter from the side opposite the main entrance.

How do I avoid getting lost in institutional POIs?

Mark your entry point on the map, close doors behind you, and place visible markers at the entrances of cleared rooms. Always maintain awareness of which direction you entered from.

Can I clear an entire high-risk POI in one run?

No. Plan to clear one wing or section per run. Attempting to clear the entire prison or sanatorium in one session consumes resources and attracts more zombies than any character can manage.

What is the minimum equipment for entering a sanatorium?

Minimum: a reliable melee weapon with remaining durability, bandages for two serious wounds, and an identified secondary evacuation route. Full medical kit, flashlight, and ranged weapon for emergencies are strongly recommended.

How does the Build 42.20 basement system affect high-risk POIs?

Several high-risk POIs gained basements in the glow-up. Treat each basement as a separate floor with its own zombie population. Do not assume a cleared ground floor means the basement is accessible or safe.
